Limited Warranty:
MD Systems, Inc. offers its customers a 2 year limited warranty that the product is free of defects in material or workmanship if used as directed in the applicable users guide. Under this warranty, products found defective are to be returned, shipping prepaid, to the factory for repair or replacement. All warranty returns must be authorized by calling the factory's service office (614-818-3000) for a Return Materials Authorization (RMA). Reason for return must be included with the shipment along with the RMA number.

To place an order by telephone, please call 614-818-3000 and ask to order a DynEx. We can usually process your request in less than 5 minutes during our normal business hours of 8am to 5pm Eastern time. For next day delivery be sure to call before 3pm Eastern time.

To request a quote or place an order by email, fax (614-818-3182), standard mail, or, after normal business hours, leave a voice message, providing the following
information:

 Inquiry Information:
    Your name.
    Medical practice name.
    Your address.
    Your phone number.
    Your credit card information if placing an order:
        - account type [VISA, MC, AMEX, Discover]
        - account number
        - V number (on back of card, in signature area, after account number,3 digits
        - expiration date
        - exact name on the card

  Product Information (state item and quantity):
    DynEx1

    Other (explain)

  Shipping Information:
    Selected speed of shipment (next day, second day, most economical).
    Company name to ship to (if needed).
    Name of person to ship to.
    Address to ship to.
    Phone number of the location shipped to.

All shipments are made using US Express Mail or United Parcel Service.

Shipping and handling costs are based on location and select delivery method:

Inside the 48 cont. US: Next day: $35.00 Second Day: $25.00 Ground: $12.00
Alaska: Next day: $60.00 Second Day: $40.00
Hawaii: Next day: $70.00 Second Day: $50.00

All shipments outside the USA are sent via UPS Worldwide Express and costs range from $70.00 to $100.00 depending on the specific destination. International shipments, including Canada, usually involve brokerage fees and import duties (taxes) which are not included in the shipping costs. In some cases US Express Mail costs can be cost-effective and will be used if shipment tracking is available to the destination.
